3. Print the waiver below, and have it signed by a parent or guardian. These MUST be turned into Tournament HQ on game day. Athletes who do not have signed waivers will not be eligible to compete in the tournament! Note: It may be easiest for coaches to collect them on game day, then provide them to Tournament HQ when they check in.
